If you require diagnostic testing, it is best to have your physician's office call to schedule your appointment. An order from a physician is needed in order for any diagnostic testing to be performed. General x-ray services do not require an appointment.
If you are unable to keep a scheduled appointment, please call us as soon as possible to reschedule.
Since you may have questions regarding insurance coverage, imaging preparation and payment policy, you may call us directly for additional information.
Insurance plans may be changing their coverage of annual mammograms so
always check with your particular plan to ensure coverage.
On the day of your visit...
Bring a copy of your physician's order (if the office has not already faxed it to us).
Physician orders are usually NOT needed for a routine, annual screening mammogram
unless the patient is younger than 40 years old
Bring a copy of your current insurance card.
Arrive 10-15 minutes prior to your scheduled appointment time to complete paperwork.

complete this form at home and bring it with you to your appointment.
Wear comfortable, metal-free clothing. If this is not possible, be prepared to change into a gown.
If applicable, bring any prior films from your physician's office with you.
Co-payment is not required at the time of your visit, as our billing service will submit your claim
on your behalf.
Self-pay rates are available upon request for
uninsured patients. Payment is due at time of service unless other
arrangements have been approved by our billing manager.
